Please print out either our NEW PATIENT PAPERWORK or our SLEEP APNEA FORMS and fill them out at your convenience. After you have filled out the form, you can return it to us in one of three ways. Either you can return it to us in person to our office at your appointment, or you can return it by fax at 541-850-8376, or you can scan the form and email it back to us at frontdesk@drouindental.com.

Together, Dr. Drouin and Deena in our front office will partner with you to help you maximize your current insurance benefits as well as plan for the best use of your future benefits when necessary.
We accept most dental insurances, and in the case of dental sleep apnea appliances, we can also help with your medical insurance coverages.
